Keep the tablet routine when you feel sick
Taking the tablet with food when you feel sick is tempting. The UK instructions say otherwise, and food may cut how much of the dose you absorb. Ask the prescriber how your symptoms should change the dose increase plan, and talk through meals for after the waiting period.
Nausea is no sign the medicine is working. In the oral semaglutide trial, weight loss did not depend on digestive symptoms. Less nausea does not mean less benefit. Severe nausea is a problem to report.
Name the exact digestive symptom
Reflux and nausea are different problems. So are constipation and slow stomach emptying. Say whether you have burning, early fullness, vomiting, hard stools or a change in how often you open your bowels. Mention any past serious bowel or stomach disease.
The UK label advises caution with gastroparesis and does not recommend semaglutide in severe gastroparesis. Ongoing vomiting of food, a badly swollen abdomen or being unable to eat needs assessment. One episode of fullness or belching is no basis for a gastroparesis diagnosis.
What to note before a review
Smaller, slower meals are often easier when your appetite drops. Do not force down a large meal because the portion once felt normal. If a rich or greasy meal makes symptoms worse each time, write the pattern down and pick something easier while you seek advice.
Sip fluids regularly in amounts you manage. If sickness stops you drinking normally, a pharmacist or clinician will tell you whether an oral rehydration product suits you. If you have a prescribed fluid limit or serious kidney or heart disease, ask for advice for your own case. A standard daily fluid target does not apply to you.
When to get urgent care
Get urgent assessment for severe abdominal pain which will not settle, above all if the pain spreads to your back. Repeated vomiting, being unable to keep fluids down, feeling faint, confusion or passing little urine also need prompt help. A painful swollen abdomen with vomiting, or being unable to pass wind, is no routine constipation.
For milder problems which keep going, contact the prescriber before you change the dose. Explain how the symptoms affect eating, drinking, sleep and work. A plan you can keep up for the long term beats a fast climb to a higher dose followed by stopping because symptoms became too much.
For constipation which will not clear, a pharmacist will suggest a treatment once they have checked your symptoms and other medicines. A sudden jump in fibre does not suit everyone with bloating or marked digestive symptoms. Skip the one-size remedy when pain, vomiting or being unable to pass wind point to a need for assessment first.
Questions for your appointment
Ask how to keep to the fasting instructions while you manage nausea, and whether to delay the next dose increase. If you are offered another medicine for the symptoms, have its timing and interactions checked against the Wegovy tablet routine.
