What the stopping evidence found

In the STEP 1 extension, people who had taken semaglutide regained about two-thirds of their earlier weight loss in the year after stopping. The extension included 327 people. Both the medicine and the trial's lifestyle support had ended.

This is an average from one study. You will not necessarily regain the same share. No trial has shown a particular taper prevents regain either. How the study was run and the support after stopping both matter when you apply the result to yourself.

Appetite returns as the medicine wears off

Semaglutide leaves your body slowly, so changes may not start the day after your last injection. Hunger and thoughts about food may grow stronger. This can feel unsettling once you are used to the appetite support.

Plan regular meals and expect hunger to return. It is no failure of willpower. Extreme dieting makes the change harder. Focus on a pattern you can keep, enough nutrition and early help if regain or distress gets hard to manage.

Why you stop changes how you stop

Suspected pancreatitis, pregnancy or another serious safety problem calls for a different response from the planned end of a funded course. Follow the clinical advice for your reason. Never delay stopping to taper when you have been told to stop at once.

For a planned stop, talk through the timing, your current dose, your health goals and your other medicines. If you have diabetes, you may need glucose checks or changes to treatment. If you take Wegovy for heart risk or MASH, involve the relevant specialist, because weight is not the only reason for treatment.

What is known about tapering

No tested tapering schedule guarantees you will not regain weight. Some clinicians tailor a gradual change and should explain the uncertainty. Schedules from social media and stretching the gap between doses on your own are unproven.

Lowering the dose changes your exposure and may be off-label, depending on the plan. Ask your prescriber what they propose, why and how they will monitor you. Aim for a clear, planned change. Avoid a string of guesswork injections from leftover pens.

Follow-up after your last dose

Agree how often to check your weight and the point at which you will ask for a review. Track other health measures linked to your original reason for treatment. If weight returns, your options can be reassessed. Going straight back on your old dose is not the only answer.

After a long break, you may need to restart on a lower dose because your tolerance has changed. Old pens may be out of date or past their in-use period. Keep a note of your last-dose date, how you tolerated the medicine and why you stopped, so a future clinician can make an informed decision.
