What the liver terms mean
Metabolic dysfunction-associated steatotic liver disease means too much fat in the liver linked to metabolic problems. MASH, or metabolic dysfunction-associated steatohepatitis, adds inflammation and damage to liver cells. Fibrosis means scarring. The terms describe related but different features of liver disease.
Stages F2 to F3 mean moderate to advanced scarring before cirrhosis. F4 is cirrhosis, a separate stage. A scan showing fat or one abnormal liver blood test does not on its own confirm MASH at the fibrosis stage the licence requires.
What the UK product information says
The Wegovy injection SmPC, updated in September 2026, covers use with diet and exercise for adults with non-cirrhotic MASH and F2 to F3 fibrosis. The standard step-up leads to a 2.4 mg weekly maintenance dose. The 7.2 mg dose applies only to weight management in adults with obesity. It is no higher dose for MASH.
A UK licence, NHS funding and a local liver service pathway are three separate things. A weight-management service may lack the tools to diagnose or manage MASH. Ask your liver team which tests you need and how prescribing will work.
What the ESSENCE trial found
The phase 3 ESSENCE interim analysis tested semaglutide 2.4 mg in people with MASH and F2 or F3 fibrosis confirmed by biopsy. At 72 weeks, steatohepatitis cleared without fibrosis getting worse in 62.9% on semaglutide and 34.3% on placebo. These figures measure changes in liver tissue. They are not weight-loss percentages.
Better liver tissue results matter clinically. They do not prove every patient is cured or will avoid a transplant. Longer-term outcomes and how long the benefit lasts still need study. Less fat on a liver scan also differs from reversing existing scarring.
Care goes beyond the injection
Your team may look at diabetes, cholesterol, blood pressure, alcohol, diet and activity alongside liver monitoring. Other causes of liver disease need checking too. Treatment should fit your whole diagnosis. A marketing line about liver health is no basis for it.
Advanced liver disease brings its own nutrition and monitoring needs. Fast weight loss or a severely suppressed appetite is not a goal in itself. Get prompt medical help for new jaundice, a swollen abdomen, confusion, bleeding or other worrying symptoms.
Questions for your liver appointment
Ask how your MASH and fibrosis stage were confirmed, what treatment aims to improve, how your response will be measured and which clinician coordinates prescribing. Check whether any current semaglutide prescription will be replaced, so you never end up with two.
If you already take Wegovy for weight management, tell the liver team your dose and treatment history. A new licensed use is no reason to add a second semaglutide product. Ask for the evidence, your eligibility and the liver follow-up to be written down, so decisions about carrying on rest on more than the scales.
