How a stomach upset affects the kidneys
Your kidneys need a good blood supply. Losing fluid through repeated vomiting or diarrhoea, or drinking little, lowers the volume of blood in circulation and can lead to acute kidney injury. This is a separate matter from any claim of direct kidney damage from semaglutide, which is not the usual pattern.
Research on kidney benefits in selected groups does not remove the short-term risk of dehydration during illness. Both are true. A medicine may have long-term benefits in some settings, and a bout of fluid loss still needs urgent attention.
Changes to watch for
Dark urine, passing less urine, thirst, a dry mouth, headache and dizziness on standing may mean dehydration. Growing weakness, confusion, unusual drowsiness or passing almost no urine are more worrying. Judge these signs against how much you have vomited, how much diarrhoea you have had and how much you have drunk.
You may underestimate fluid loss when Wegovy dulls both appetite and thirst. Drink at regular times. Do not wait until you feel strongly thirsty. A sudden drop on the scales over a day or two is likely to be water. Do not count it as fat loss.
Practical steps to take early
If your illness is mild and you can drink, regular small sips are often easier than large drinks. A pharmacist can recommend an oral rehydration product where suitable. Mix the product as the instructions say. Extra powder or a home-made strong mix can be unsafe.
People with heart failure, advanced kidney disease or a prescribed fluid limit need individual advice on fluids. A blanket instruction to drink several litres suits only some people. Contact the team managing your condition when illness upsets your usual plan.
Your other medicines
Diuretics, some blood pressure medicines, anti-inflammatory painkillers and diabetes medicines all matter during dehydration. Do not draw up your own sick-day list or stop every prescription at once. Ask a clinician which medicines to pause, and exactly when to restart them.
If you use insulin, do not assume you can stop because you are eating less. You may need to check glucose and, where relevant, ketones as your diabetes plan sets out. Tell each team you are ill while taking Wegovy so they can join up their advice.
When to get urgent care
Get urgent advice if you keep vomiting and cannot keep fluids down, still show signs of dehydration after rehydrating or pass much less urine. Confusion, collapse, severe breathlessness or severe tummy pain may need emergency care.
Contact your Wegovy prescriber before the next dose increase if you have eaten and drunk poorly. You may need blood tests, a slower dose schedule or a different treatment. If treatment stops for a while, a safe restart depends on how long the break lasted, your previous dose and your recovery. A spare pen in the fridge is no guide to the right dose.
